never really had a problem with any of the burst components, but we have had a number of brand new FA's with safety issues. the failure happens when the selector somehow gets placed half way between auto and semi and the trigger is pulled and released but the hammer doesn't fully catch, once...
they have been 3 round burst for quite a while, they are in process of replacing many with full auto now, which I personally think is a mistake.
I'm one of the 91F (small arms and artillery repair) for my unit and we just replaced all our burst models with full auto M4's
