    Nevertheless, if the post-program incident rate is greater than zero, that may not necessarily mean any given prevention program a total failure. While there may be more than zero incidents despite the program, perhaps without the program the incident rate would have been five or more times greater. If a program reduces but does not eliminate bad incidents, can it be branded a complete failure? Lessons should be learned and improvements made. Failing to do this would be a failure.

    This part of the MoCO360 article seems important:

    "On March 3, Ye sent a 129-page “fictional story/manifesto about a high school shooting” to someone they knew when they were undergoing inpatient treatment at a psychiatric facility, according to charging documents. The documents refer to the individual as “Witness-One.”"

    Indeed. Closing the loop seldom occurs. Introspection may disclose the need for change, mild to radical, or elimination, all of which is painful to the infrastructure that sprung up to address the issue and must be avoided.
